Weight-Loss Surgery Eligibility
Managing health conditions related to weight can be frustrating, even on the best days. Bariatric surgery offers a powerful, evidence-based solution when other methods fall short. But are you a candidate?
According to URMC specialists, eligibility hinges on body mass index (BMI), obesity-related health issues, and emotional readiness. Learn more about the requirements for bariatric surgery, plus how to self-assess.
What Are the BMI and Health Requirements for Weight-Loss Surgery?
Most insurance companies and medical guidelines consider you eligible for weight-loss surgery if:
- Your BMI is 40 or higher, or
- Your BMI is 35–39.9 plus you have a related condition like diabetes, high blood pressure, or sleep apnea
Other qualifications include:
- At least 18 years of age
- Attempts at conventional weight-loss programs (like Atkins, Nutrisystem, or working with a registered dietitian)
What is BMI?
Your Body Mass Index (BMI) comes from a formula based on your weight and height. This number is also used by insurance companies to determine if you are eligible for weight loss surgery coverage.
- The ability to follow pre- and post-operative instructions for diet and exercise
- No illegal substance use in the past year
Why Does Psychological Readiness Matter in Bariatric Surgery?
Weight-loss surgery isn’t just about the procedure—it’s committing to a life-long lifestyle transformation.
Your care team will want to make sure you’re emotionally and mentally ready for what comes after.
You may be asked about your relationship with food, your history of weight changes, your coping strategies during times of stress, and whether you’ve experienced depression, anxiety, or substance use. These questions are not meant to disqualify you but rather to identify any support you might need as you move forward.
Signs of emotional readiness often include:
- A clear understanding of what surgery involves and realistic expectations about outcomes
- Willingness to follow medical advice and attend follow-up appointments
- Openness to making long-term changes to eating and activity habits
- A support system of family, friends, or caregivers who can help you through recovery
Addressing mental health is a key part of setting yourself up for long-term success. If more support is needed before surgery, your care team will work with you to make a plan that’s right for you.

Bariatric Surgery Eligibility Self-Assessment
- Calculate your BMI.
- Evaluate obesity-related health conditions like high blood sugar or high blood pressure.
- Reflect on mental and emotional readiness:
- Have you tried dieting and exercising consistently for at least 6–12 months?
- Do you have support from friends or family?
If you answered “yes” to all three, you may meet bariatric surgery eligibility criteria.
